Community Health - Asthma Quality Improvement Project

Asthma makes up 10% of UPMC Children’s Hospital of Pittsburgh’s hospitalizations and a similar percentage of Emergency Department visits. Some neighborhoods in Pittsburgh with concentrated disadvantage have particularly high rates of Emergency Department utilization. This project aims to connect kids and families with asthma to community-based resources and follow-up care.

UPMC Children’s Family Care Connections nurses or community partners in seven Pittsburgh neighborhoods are following up with families after their child is discharged from the Emergency Department for asthma to offer resources and connections to community-based care. Families are followed up with one month later to see how their child is doing, whether they followed up with a PCP, and whether they utilized the resources.
